The representative must be able to represent the following: In cases wherein, the claiming customer cannot personally claim his/her Free premium items or any reason, he/she can send a representative in his/her behalf.Photocopy of any two (2) Valid Forms of identification such as SSS card, TIN card, Drivers License, Voters ID, Passport or Senior Citizen ID.In claiming the Free premium items, the following must be presented to an AOC Monitor Personnel (Promoter).The proof of purchase must have the quantity and model unit of participating AOC monitor.Customer’s copy of Official Receipt or.Upon claiming the FREE premium items, the following are valid and accepted proof of purchase:.Claiming customers will only get the corresponding premium items and quantity base on the proof of purchase.The costumer may claim the premium items after purchasing a participating product.Premium items are not convertible to cash.Customer can only claims his/her FREE premium items where he/she bought the AOC monitor.Multiple claims with one proof of purchase will be honored provided all unique serial numbers of purchased product is provided upon claiming.One proof of purchase with bulks orders will be valid for multiple claims done on a one-time basis only.Only one person can submit a claim per item purchased.Multiple claims for a single item purchase are not allowed.One monitor purchase is entitled to claim premium items accordingly.For every purchase of any participating AOC Monitor customer entitles to a premium items.
